We can create Custom case form and also fetch custom case form values to template.
Step 1: Setup -> Support -> Online Case Forms -> New .
Step 2: Select the ‘Cutsom HTML Template’ from the list.
Step 3: In ‘Cutsom HTML Template’ window, here we can set the Title and update the required fields from the list. After that click on the ‘+’ button at the right side of the template.
Step 4: In ‘Online Form Template’ here we set the name for template, choose the ‘File’ and select the ‘New’ option.
Step 5: Here we selecting the email template from our system. After that click the Save button.
Step 6: Follow the ‘<NLFIRSTNAME></NLFIRSTNAME>’ format for accessing the selected fields in template.
Step 7: Click the Save button in ‘Online Template Form’ and ‘Online Case form’
