Other record field created for address was not visible in address book because the country was not set in the selected address forms.
Navigation to create other record fields is,
Customization -> Lists, Records and Fields -> Other record fields
We need to select the countries for which the other record fields created are to be applied.
Navigation to select the address forms,
Customization -> Forms -> Address forms
