Proposal For Appointment Report and CRM Report- Phase 1

PROPOSAL SUMMARY 

This proposal outlines the scope of Appointment History Detailed Report , Month wise Appointment Report and CRM Report 

REQUIREMENTS  

1. Appointment History Detailed Report 

NewLife Fertility Centre needs to create Appointment report which should have the below structure. The report is used to show the details of in each date how many number of appointments are there for each appointment type in each location. 

2. Month wise Appointment Report 

NewLife Fertility Centre needs to create Monthly Appointment report to show the details of in each Month how many number of appointments are there for each appointment type in each location. The column of this report should be the same as the above report. 

3. NLFC CRM dashboard (arfah/appointment) report 

NewLife Fertility Centre needs to remove the duplicate customer line from the NLFC CRM dashboard (arfah/appointment) report. 

DELIVERABLES 

• We will create a saved search report to display the number of appointments are booked for each appointment type in each location. 
• One report will display each date’s details, while another will display data by month. 
• Following columns will be included in the both report 

• The value will be reported as 0 if the location has no appointment types. 
• We will exclude the duplicate customer line from the “NLFC CRM dashboard (arfah/appointment) report” and only display the most recent task that was created for that client. 
• We will add all the columns and filter into the report which would be the same as the exist in the report. See the image below for references

ASSUMPTION 

• The Appointment History Detailed Report and Month-wise Appointment Report are the information from the  Record of the ‘Appointment History Record’. We will not consider any other record for these reports. 
• The report (both Appointment History Detailed Report and Month-wise Appointment Report) only shows the past 12 months and future 12-month data’s from at present dates. 
• The list of appointment types and locations will provide by the NewLife Fertility Center. 
• If NewLife Fertility Centre needs to add new column on this report it will be consider as the new update of the reports.
