This proposal included the requirement and solutions regarding the updates in login page,
footer and addition of new “TERMS AND CONDITIONS” static page
Requirement
- Add “TERMS AND CONDITIONS” static page – add link under the “PLATINUM NORTH
AMERICA” category - Add an online lead form on the login page
- Need to update the copyright date in the footer to 2021
Our Solution
1. “TERMS AND CONDITIONS” static page
➢ A new landing page in SMT
➢ The UI of the page will be the same as the page “about-us”
➢ Link for the page will be added under the list of “PLATINUM NORTH
AMERICA” category in footer section
2. FORM in login page
➢ We create a new registration form
➢ Fields in the form :
○ Contact Name
○ Company Name
○ Email Address
○ Phone number
➢ When Submit Form sends an email to info@platinumna.com with the following details

The form has been submitted with the following details
Contact Name :
Company Name:
Email Address:
Phone number:
➢ A message will be displayed on the website after the form is submitted as
Thank you for registering
➢ When there is a problem with the form submission the message shown as
There was an error submitting the form, please try again later
➢ A lead will be created (Individual type) and maps the details to the following fields.
○ Contact Name -> NAME
○ Company Name -> COMPANY NAME
○ Email Address -> EMAIL
○ Phone number-> PHONE
➢ Remove the text “Login”
➢ Change the text “Returning customer” to “DISTRIBUTOR LOGIN”
3. Copyright date in the footer
➢ Update the year into 2021 using an extension
➢ The extension will be updated automatically each year. No need to change in the coming years.
Assumptions
We can use the existing Netsuite registration form if we add the password section in the form.
