Navigate to Setup > Users/Roles > Manage Roles Click Edit/Customize by the Role for which the custom form will be restricted Click Forms tab Click Transaction subtab Find the forms with a Opportunity Type Preferred: Enter Checkmark by the form you want to restrict for this Role Enabled: Remove Checkmark for other Opportunity forms Click Save
